Draft Family Law Forms

The Kentucky Court of Justice is asking for input on legal forms for use in family law cases.

The forms for review are for cases involving domestic relations, status offenses, and dependency, neglect or abuse. In addition to being available for comment, the domestic relations forms are in the pilot stage and may be used. The new/revised forms on status offenses and dependency, neglect or abuse are being released for comment only and are not intended for use at this time.

Also up for consideration are technical corrections to the Family Court Rules of Procedure and Practice regarding forms.

Please send comments and questions to FCRPPforms@kycourts.net. The deadline to submit feedback is Thursday, July 31, 2014.
